Social care gains from Telehealth interventions

Telehealth in many areas to date has focussed purely on the management of specfic disease areas, such as COPD, and the business models have looked at savings such as reduced admissions into Hospital, which have proven to be hard to measure. However, if applied to the whole aspect of a clients needs, both health and social, the quality of life is significantly improved and savings are far higher and measureable, especially when looking at preventing admission to a residential or nursing home.
This paper will present several case studies from sites in the UK where social and health care work closely together under joint commissioning and where client care has been optimised and improved by the use of carefully targetted and customised use of the Docobo telehealth system
